(likes + comments + shares) / followers × 100. If you know impressions, you can also calculate it against impressions for a per-post view.

It depends on your follower tier. Under 1,000 followers, 4-8% is strong. Over 100,000 followers, well under 1% is still healthy. Compare yourself to your tier, not to viral outliers.
